A Hawaii 45 Day Notice to Quit is a lease termination notice for tenancy-at-all and month to month rental agreements. The purpose of this document is to inform the tenant that the landlord has made a decision to stop renting out their property to them. This notice does not require a landlord to disclose a reason, but may require rent to be prorated depending on the last date of tenancy.

How to Fill Out
Step 1 – Complete the first and last name of the tenant(s).
Step 2 – Identify the rental property by its address.
Step 3 – Enter the last day, month, year and time when the tenant must vacate.
Step 4 – After the tenant is served, the person who delivered the notice must reveal how and when it was delivered by completing the Record of Service section.
