Any tenancy with or without a monthly rental agreement that is paid month-to-month, must be terminated with a Massachusetts 30 Day Notice to Quit. This rental form gives ample time required by law to end a landlord-tenant relationship. If the tenant does not move out by the date specified in the form, then the landlord may proceed with a summary process to recover possession.

Step 1 – Complete the name(s) of the tenant(s).
Step 2 – Enter the rental property address.
Step 3 – Include the date and time by which the tenant must vacate.
Step 4 – Sign and date.
Step 5 – Serve the tenant with the notice and complete the record of service section stating how and when the tenant was served.
